AS DIRECTED BY PUBLIC LAW 106-119: - Title 22, Section 51.28
Effective February 1st, 2008
To submit an application for a child under age 16
both parents or the child’s legal guardian(s) must appear and present the following:
Evidence of the child’s U.S. citizenship
(an original birth certificate or an original naturalization certificate)
If the child has an expired passport – they will submit their expired passport as well as their birth certificate – this will show relationship to the parents
Parental identification
If only one parent appears
you must also submit one of the following:
The non-attending parent must fill out consent form (DS 3053)
and it must be NOTARIZED and submitted with the application and the applying parent will sign the application in front of the clerk.
Primary evidence of sole authority to apply OR
A written statement (made under penalty of perjury)
explaining the second parent’s unavailability.
When applying for a U.S. Passport bring the following items:
Proof of U.S. citizenship, such as an original or certified birth certificate Original Naturalization Certificate
Expired U.S. passport
Proof of Identity - Current photo ID such as -
Valid Driver's license or State Identification Card
2 recent, identical, color photographs
- Normal (routine) processing takes 4 to 6 weeks.
The total for an Adult Passport (16 years or older) is $100.00. This is payable in TWO separate checks, money orders or travelers checks. The first of two checks is payable to the U.S. Department of State in the amount of $75.00. The second check is payable to the City of Coral Springs in the amount of $25.00.
(Only the $25.00 fee can be paid in cash)
The total for a child's passport (15 years and under) is $85.00. The first of two checks is payable to the U.S. Department of State in the amount of $60.00. The second check is payable to the City of Coral Springs in the amount of $25.00.

To receive your passport quickly you must pay an additional $60.00 more per person.(this is in addition to the passport and execution fees above)
U.S. Express Mail is also available charges are based on current postal rates, payable by check to the City of Coral Springs.
PLEASE NOTE: If you are paying by a personal check – your personal check must have the following: payer’s name and address PRINTED on the face of the check. If you do not have this on your check you must bring in a money order.
NO STARTER CHECKS WILL BE ACCEPTED.
You may go to the Website www.travel.state.gov and download the passport application DS11 and the consent form DS-3053 (if one is needed)
NEW PASSPORT CARDS
The Passport Card may ONLY be used for land and sea travel between the United States and Canada, Mexico, Bermuda, and the Caribbean.
Customers will now have the option to apply for a Passport Book, a Passport Card or Both.
TO APPLY FOR THE PASSPORT CARD YOU WILL STILL NEED TO SUBMIT THE SAME DOCUMENTS THAT ARE REQUIRED FOR A U.S. PASSPORT BOOK.

Adult Passport Card Fees:

This form may ONLY be used if YES is answered to ALL four questions.
- I can submit my most recent U.S. passport
- I was at least 16 years old when my most recent U.S. passport was issued
- I was issued my most recent U.S. passport less than 15 years ago
- I use the same name as on my recent U.S. passport
- OR -
I have had my name changed by marriage or court order and can submit proper documentation to reflect my name change.
If you answered NO to any of the four statements above, STOP -
You cannot use this form!! You must apply on application DS-11 by making a personal appearance before a passport agent, postal clerk or clerk of court authorized to accept passport applications.
